The saturation of hospital emergencies is a worrying phenomenon that affects the efficiency of health services, access and the quality of care provided to patients, having demonstrated a negative influence on patients' health. A retrospective longitudinal study was carried out coordinated between twenty-three Health Centers in Tenerife with patients discharged from the Canary Islands University Hospital between January 1, 2018 and December 31, 2022, using the visits they made to the emergency services as an effect of interest. hospitalized for different reasons in the 6 months after discharge. We identified the main risk factor for visits to hospital emergency rooms as the lack of planned attendance at the first consultation with the EFYC. Easily modifiable factor through a minimal intervention strategy that plans the visit to the EFyC at the time of hospital discharge.
