Desplazamiento de catéteres venosos de punción ecodirigida. Estudio transversal sobre la influencia de la estrategia eFIX para la selección del dispositivo de fijación

Resumen
Abstract: INTRODUCTION: Ultrasound-guided micropuncture cannulation of deep veins of the arm increases the success rate at the first attempt and reduces the probability of complications, which results in an improvement in the efficiency of the procedure. Displacement of the catheter is an avoidable complication that is related to a higher risk of phlebitis, thrombosis, and infectionsb as well as greater financial costs and, therefore, a decrease in the efficiency of the procedure. OBJECTIVE: To analyze the influence of the "eFIX" fixation device selection strategy on the displacement of venous catheters implanted with ultrasound-guided puncture. MATERIAL AND METHOD: A cross-sectional analytical observational study was carried out. It included all patients older than 14 years admitted to the University Hospital of La Palma who met the criteria for a Midline (ML) or Central Peripherally Inserted Catheter (PICC) between July 1, 2021 and June 30. 2022. Displacement of the catheter was identified as a dependent variable and the application of the self-made fixation system selection strategy (eFIX) as an independent variable, which includes the routine use of adhesive devices (AD) with statlock technology and the selection of patients for the use of subcutaneous anchors (ASC) and tissue adhesive. Data analysis with descriptive statistics with OR effect measure. RESULTS: 378 records were reviewed. 171 (45.23%) correspond to the group without application of eFIX, identifying 25 cases (14.61%) of displacement and 207 (54.76%) belonging to the group with application of eFIX, identifying 11 cases (5.31%). ) of displacement. Resulting in an OR= 0.32 with a standard error of 0.37 and a 95% CI (0.68 and 0.15). CI 99% (0.86 and 0.12) In the first group, 7 cases (4%) of thrombosis, 1 case (0.5%) of phlebitis and 0 cases of infections were identified. In the second group, 7 cases (3.38%) of thrombosis, 0 cases of phlebitis and 0 cases of infections were identified. DISCUSSION: The data show a decrease in catheter displacement in patients with eFIX application. It seems that the application of this strategy influences the decrease in displacement by discriminating the use of AD and ASC based on the low risk of displacement attributable to ASC (0-1.5%) f compared to the 14-20% attributable to DAe. The study may be limited by selection bias and confounding, but the results open the door to future studies. CONLUSION: The application of the fixation system selection strategy was considered an influential factor in reducing the prevalence of displacement of venous ultrasound-guided puncture catheters and related complications.
