Abstract

Case report: 72-year-old female patient with a personal history of type 2 diabetes and hypertension. The patient presents acute unilateral latent conjunctivitis in her right eye that does not improve after topical treatment with dexamethasone and chloramphenicol. The exploration reveals scleritis in her left eye. An autoimmune process is discarded and treatment is started with oral and topical corticosteroids, without improvement. Besides, the patient deteriorates with vitritis. Antibiotics and antifungal intravitreal injection is prescribed, and a conjunctival biopsy requested. Findings show infiltration consistent with parasitic helminth, PCR amplification reveals no positivity for the most frequent helminths. After treatment with mebendazole, topical and oral corticosteroids, the process improves progressively. Conclusions: Parasitic scleritis is a rare condition, and it should be suspected when other conditions are discarded and when the prescribed treatment does not show good results. This case has been solved thanks to multidisciplinary work.
